Scientist Emeritus Agreement Form and Annual Work Plan

N.B. if you use a Mac or other operating system, you may need to have the Help Desk reactivate your Active Directory account. - If you can not access the Scientist Emeritus Annual Workplan system, please contact the Help Desk and tell them that you need to reactivate your Active Directory account. You already may have an Active Directory account and not know it. The Help Desk will be able to determine this and, if you already have an account, they will be able to help you access it and will reset the password for you. If you do not log into the USGS network/domain but have a Lotus Notes account or a "usgs.gov" e-mail address, please let the Help Desk know when you contact them.
- If the Help Desk determines that you do not have a Lotus Notes account/usgs.gov e-mail address, please provide a digital version of the information requested under “Instructions for completing your Scientist Emeritus Annual Work Plan” below to your Science Center Director and to Judy Back. The file will then be attached to your on-line Volunteer Service Agreement.

Ethics: Scientists Emeriti are expected to follow ethics rules applicable to their status and position. Written agreement to abide by the Ethics Rules for Scientists Emeriti, http://geology.usgs.gov/Emeritus/about/policies/ethics.shtml, is an expected "condition of acceptance" into the Emeritus Program and continued adherence to them is a “condition of continuation” in the Emeritus Program.
Agreement form and work plan
For fiscal year 2010, we will use the new on-line Volunteer Services Agreement form, https://gsvaresa08.er.usgs.gov/Webforms/OF-301A.nsf. The new form will need to be updated annually to reflect any change in status and add hours worked and expenses incurred the previous year (by quarter).
Annual work plans should be entered on the new on-line site, http://130.11.48.100/fmi/iwp/cgi?-db=Seawus&-loadframes. When you submit your work plans on this site, you will automatically receive an e-mail with a PDF file of your work plans that needs to be attached to the bottom of the continuation form of your Volunteer Services Agreement.
Complete your annual work plans before your Volunteer Service Agreement form.
Please follow the instructions below carefully.
Instructions for completing your Scientist Emeritus Annual Work Plan

- Log into the system. To log into this system, use your Active Directory account (the same id and password you use if you log into the USGS network/domain from a Windows computer each day. - Enter appropriate fiscal year from pull-down menu at the top of the page.
- Complete or edit, if necessary, your personal information (name, address, phone number, preferred e-mail address).
- Indicate whether or not you will apply for Bradley funding.
- The gray box at bottom of page includes six tabs for you to complete. Please note that you can copy and paste text into the fields on these tabs.
- Work plan: Please discuss your work plan, including for the upcoming fiscal year with your Science Center Director and enter below. Please indicate all of the places where you will be working (e.g. , field, home, USGS office, overseas, etc). This is essential in the event you incur an injury while performing your emeritus work.
- Projects: Please list all USGS projects with which you will be participating in the upcoming fiscal year. Please include the Project Chief's name.
- Funding: Please discuss your funding needs for the upcoming fiscal year with your Science Center Director and list them in this field. Please do NOT include Bradley proposal funding requests. Include how the money will be used: publication costs, purchase of maps, field work, laboratory expenses, attend meetings, drafting, GIS, etc. To add a second budget item, simply click the green “Add” button on the left side of the field.
- Accomplishments: Please discuss your recent Scientist Emeritus accomplishments. Manuscripts in progress can be listed in this field.
- Publications: All Scientist Emeritus publications will be listed on the Scientist Emeritus Web site, http://geology.usgs.gov/Emeritus/. Please list all of your Scientist Emeritus publications that are not already listed on the Web site. To add a second publication, simply click the green “Add” button on the left side of the field.
- Non-Publication Products: Please list any recent non-publication products completed as a Scientist Emeritus, e.g. exhibits with partners, educational materials, etc. To add a second product, simply click the green “Add” button on the left side of the field.
At the end of a session, please make certain to:
- Save changes. Please press the top pink “Save Changes” button on left side of page
- Exit system. Please press the bottom pink “Exit” button on left side of page when you want to complete a session, but have not finalized your work plans.
When you have completed the form, please press the middle pink “Submit Work Plan” button on left side of page. This will result in your plans being saved (as a PDF file) and e-mailed to you (at the e-mail address you indicated on this form).

The on-line Volunteer Services Agreement, https://gsvaresa08.er.usgs.gov/Webforms/OF-301A.nsf, is accessible using your Lotus notes ID and internet password. If you don't know your internet password, you can either call the Help Desk at 703-648-7300 or email them at servicedesk@usgs.gov.

- Section B. Indicate whether you are a Scientist Emeritus or Honorary Scientist Emeritus.
- Section D. Please keep track of your hours and expenses. After each quarter, make certain to add them to this section of the form.
- Continuation page tab (do this last)
- At the bottom of this page, please attach the PDF file of your Scientist Emeritus Annual Work Plan. The form currently has room to hold 5 years worth of work plans.Please note, there is no limit to the number of hours or years you can serve as a Scientist Emeritus.
- After you have attached your work plan, please go to the top of the page and hit the “Save” button before you do anything else. Once you hit the “Save” button, you will leave your form. To re-enter the form, please:
- Press the “VIEWS” link
- Click on either the “Date Created” or “Volunteer” link.
After you have successfully attached your work plan, it will appear as a link and attachment icon at the very bottom of the Continuation page. If you attached the wrong file and would like to delete it, check the box to the left of attachment and then hit the “Save” button at the top of the page. WARNING: If you hit the “Delete” button, you will delete your entire Volunteer Services Agreement form!
